Insurance Coverage Accepted
- Medi-Cal: Care 1st Health Plan, Community Health Group, and Molina Healthcare.
- Covered California: Plans from Blue Cross, Blue Shield, Health Net, and Molina Healthcare.
- Private Insurance: PPOs.
If you don’t have health insurance, our certified enrollment counselors can assist you in enrolling in a low or no-cost health plan through Covered California or Medi-Cal. We also help current Medi-Cal recipients transition into one of the managed care plans accepted at our center.
Financial Assistance and Required Documentation
For uninsured patients with income up to 200% of the federal poverty level, we offer a sliding fee scale that bases medical service costs on family size and income. To complete an application, you will need to provide:
- A California driver’s license or state identification card.
- Proof of permanent residency or certificate of citizenship (if born outside the United States).
- Proof of all income (such as recent paycheck stubs or a letter from your employer).

Services for Verified American Indians/Alaskan Natives
Verified American Indians/Alaska Natives can receive the following services at no cost:
- Medical office visits
- Behavioral health visits
- Routine dental care (including exams, x-rays, cleanings, and fillings)
Please note that while these visits are covered, there may be costs for labs, x-rays, medications, or referrals not included in special programs.
